Baltimore bridge collapse

A single misinstalled wire set off the catastrophic power failure that caused Baltimore’s Francis Scott Key Bridge to collapse in March 2024, the National Transportation Safety Board (NTSB) revealed on Tuesday. The new report confirms that the tragedy, which killed six construction workers, was “entirely preventable.”

The container ship Dali crashed into a support pier after experiencing two major blackouts within minutes. Investigators now say those failures came down to one faulty wire label, years of overlooked safety requirements, and inadequate oversight from the ship’s operator and Maryland authorities.


What Caused the Crash?

The NTSB found that an improperly installed signal wire loosened over time and disconnected from its breaker. That small fault triggered a total loss of propulsion and steering. It left the 947-foot vessel drifting straight toward the Key Bridge with no way to stop.

NTSB Chair Jennifer Homendy said locating the faulty wire was like “finding a loose bolt in the Eiffel Tower.” Yet she stressed that proper inspections should have caught it years ago.

The ship suffered two blackouts the day before the crash while docked. Investigators said the crew used a flushing pump incorrectly as a fuel service pump, a practice Synergy Marine Group failed to stop. Because that pump could not restart automatically during blackouts, the Dali could not recover power in time.


Crew Reacted, But Had No Chance

The report notes that the crew acted quickly, but the vessel was already too close to the bridge. Seconds before impact, power flickered, lights cut out, and black smoke rose from the funnel — all signs of cascading electrical failure.

All six road workers on the bridge died in their vehicles. They never received a timely evacuation warning due to communication delays.


Maryland Also Shares Blame

While the ship’s electrical faults triggered the crash, the NTSB says Maryland’s Transportation Authority (MDTA) failed to perform a vulnerability assessment that would have identified the bridge’s extreme susceptibility to a ship strike.

The report states that the bridge lacked modern countermeasures that could have prevented total collapse, such as reinforced fenders or protective structures.


Cost to Rebuild Has Exploded

Maryland officials now estimate the replacement bridge will cost $4.3 to $5.2 billion — more than double early projections. It will also take two additional years to complete, with a new opening date set for late 2030.

Inflation, supply chain pressures, and steel prices are major contributors to the rising bill. Officials say rebuilding the bridge remains one of the most complex infrastructure projects in state history.


The Bigger Picture

The tragedy highlights ongoing concerns about aging U.S. infrastructure, ship safety standards, and rising maritime risks. Investigators say the disaster resulted from a chain of preventable failures:

  • A mislabeled wire during construction
  • Poor operational oversight
  • Misused critical equipment
  • Failure to follow long-standing safety recommendations

As the state works toward a rebuild, the full cost — human and financial — continues to grow.
